Vision or Hearing Exams
Some medical forms may require a separate vision or hearing exam. Where required, these should be completed before the appointment so the results can be reviewed during the assessment.
Medical Assessment Requirements
To complete a medical assessment, the clinic will require proof of ID, such as a valid passport or driving licence, along with a copy of the patient’s GP summary printout.
A GP summary can usually be obtained from the patient’s usual NHS provider. Reception teams at NHS practices are often able to provide this on request.
